HEALTH EQUITY INITIATIVES: In response to systemic fragmentation throughout Alameda County, we works to bridge gaps and facilitate communication among the participating community organizations and agencies in the ACLCC and Sobrevivir, as well as inside ACMC/Highland's Oncology, Hematology-Oncology, Breast, and Infusion Clinics and the Internal Medicine Department through active Volunteer Patient Health Advocate/Navigation Services.
